Access to Medical Records

The practice offers the facility for patients to view online a summary of their detailed coded Medical Records, including Medication, Allergies, Laboratory Test Results, Problems, Immunisations and Adverse Reactions.

To register you need to present in person at the Health Centre:

  • Two forms of documentation as evidence of identity, one of which must contain a photograph. Acceptable documents are Student ID Card, Driving Licence, Passport, Bank Statements or a Utility Bill.
  • Complete an Application Form. This will be forwarded to a GP for authorisation prior to access being granted. Please allow a minimum of 7 days for this to be actioned.

The practice is registered and complies with the Data Protection Act 1998.

Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act.
