| When you have completed your Registration with the Health Centre, you will need to complete a Health Information Questionnaire. This will ensure that your doctor has some basic health information about you and will be better able to advise, diagnose and prescribe treatment for you - especially if we have yet to receive your medical records from your previous GP. | |
