| REPEAT PRESCRIPTIONS

Your doctor may issue a prescription for medication for your medical condition and you take this script to a Pharmacy / Chemist shop to be issued. If you need to take this medication over an extended period of time then the doctor may authorise repeat prescriptions. Requests for repeat prescriptions can be made:
- In person at the University Health Centre by using the computer slips attached to your previous prescription or by completing a request slip at reception
- by telephone: (024) 765 74406 or Internal extension 74406 between 10am - 12 noon and 2pm -4pm only. This number is for repeat scripts only Please ensure you have the name of the medication(s) you require with the strength and dosage. We will endeavour to answer promptly but bear with us if there is a delay when we are busy.
- ONLINE via this website or email. We need your full name, address, date of birth, the practice you are registered with & details of the medication(s)
Allow at least 48 hours (in working week) before collecting the prescription. Unless you have obtained an exemption (see form HC1 or ask reception) you will be charged £7.10 per item at the Pharmacy. Normally we will only give the prescription to the person named on the script. If you will be asking a colleague to collect on your behalf please tell us in advance. We will post prescriptions if you supply a stamped self-addressed envelope. When requesting a repeat prescription of contraceptive pills please note that you may need to see the nurse prior to receiving the script - check & organise an appointment if needed.
It is important to request regular medication at least a few weeks before you run out particularly if this is the first time this GP practice has issued it or you are about to depart from University over a vacation period. You may be entitled to exemption from prescription charges. An application form is available from reception and advice may be obtained from your Student Union Welfare Officer. |

How
do I - order a REPEAT PRESCRIPTION?

| There are three
main ways in which you can order a repeat prescription from us. Whichever
method you choose to request a repeat prescription please allow
48 hours from when we receive it before collecting this from our
reception desk. Normally we will only give prescriptions to the person
named on the script. If you will be asking a colleague to collect
on your behalf please tell us in advance. Please note that if you
order a repeat prescription on a Friday it will not be available until
Tuesday. If you e-mail over a weekend we will not pick these up until
Monday and they should be ready on Wednesday. |
| When you receive your prescription
you will need to take it to a Pharmacy to be issued with your medicine(s).
There will be a charge for each item issued unless you have an exemption
certificate. |

To ensure your treatment is kept up-to-date you may
be asked to see the doctor before another repeat prescription is
issued.

Using your tear off slip
which is attached to your prescription that you collect from us. Please
indicate clearly on this form which items you wish to order on repeat
prescription and either put this in the box situated just inside our
reception OR post it to us. (If you wish us to post the Prescription
back to you then you must enclose a stamped, self-addressed envelope)
if you have lost or mislaid your re-order slip then please
fill in the form which you can find just inside our reception. If
you have any problems consult one of our receptionists.

During our open hours you
can telephone us on (024) 7652 4888 after 10 am. The reception
area is very busy and the University cannot provide us with additional
telephone extension lines so our apologies if you have to try to telephone
more than once. It is usually quieter after 11am and in the afternoon. |
