OF ADDRESS
Both GP
practices provide general medical services to patients who are registered
with one of the practices and who are resident in the catchment area
for that practice. Broadly this covers University students and
staff living on campus or in a defined area of south and west Coventry.
The exact area is displayed on a map in the Health Centre (or
click here).
Unfortunately we cannot provide medical services to persons living
outside this area and it is important if you move (e.g. to Leamington
Spa ) that you register with a new GP covering the area where you
are resident. You will encounter difficulties if you fail to do
this and need to see a doctor at home or if you are ill when the
health centre is closed.
It is also important to tell us if you change address within
our catchment area e.g. change rooms or flat on campus. The
Primary Care Trust regularly checks our lists and if they suspect
a person is no longer resident at the address they are registered
then they will remove the patient's name from our list and your
medical notes have to be returned to the Primary Care Trust.
If away
from University for the vacation you do not need to either "change
address" or doctor. If you are ill you will be able to see a
GP as a temporary resident in the UK location in which you are staying.

and giving it to the receptionist

your old address and postcode, your new address and postcode, telephone
and mobile phone number and you e-mail address
